Chromebook Insurance Enrollment Form
Optional Insurance Program for Families
Coverage and Benefit:
This program is designed to protect students and families in the event that the Chromebook issued to a student is damaged or malfunctions due to no fault of the student. Students and parents may be charged for any intentional device damage, which is not covered by the policy.
If a family does not enroll in the program and pay the annual premium by September 30, 2025, the family will be responsible for the full cost of repair, including labor or full replacement cost of the Chromebook or charger resulting from any damage or loss.
Families experiencing economic hardship may request a fee waiver by contacting the school administrator.
The policy covers the student’s ER9-issued Chromebook against accidental damage to the Chromebook or theft of the device subject to the following:
First Incident of Damage: 100% of the cost covered
Second Incident of Damage: 50% of the cost covered
Third Incident of Damage: 25% of the cost covered
The cost for the 2025-2026 school year is $45.00. In the event that a student withdraws or enters the district at any time during the school year, the amount will not be prorated and is not refundable.
Effective Dates:
Coverage is effective from the first day of the school year until the Chromebook is collected at the end of the school year. Coverage must be renewed annually.
**Chromebook insurance enrollment is per device. If you have multiple children at HKMS, you will need to purchase Chromebook insurance for each child separately.
